OBSTETRICAL-GYNECOLOGICAL PATHOLOGY, ENDOCRINOLOGY AND SEXOLOGY
(objectives)
At the end of this course, students will be able to describe the major complications of pregnancy, labour, and delivery, explaining their prevention, diagnosis, and treatment. They will be able to identify the most prevalent endocrine and metabolic disorders based on their clinical features, knowing the main laboratory and imaging tests required for the diagnostic procedure. Moreover, they will acquire the basics of Human Nutrition, obtaining a functional understanding of Nutritional guidelines during pregnancy and lactation and awareness of the nutritional aspects of maternal and child health on a Global Health level.

The indispensable learning objective is the knowledge of the major complications of pregnancy. Particular interest is payed to: 1) The most relevant complication of the 2nd trimester of pregnancy: • cervical incompetence 2) The most relevant complications of the 3rd trimester of pregnancy: • Hypertension in pregnancy (gestational hypertension, preeclampsia, eclampsia, preexisting hypertension) • Diabetes in pregnancy • Preterm labor • Placenta previa • Placental abruption • Intrahepatic cholestasis of pregnancy • Virali infections in pregnancy: HIV, COVID-19, HAV, HBV, HCV, ZIKV • Puerperal disorders 3) Other issues • Thyroid problems in pregnancy • Polyhydramnios – oligohydramnios - anhydramnios

Acquire the ability to detect and identify the most prevalent endocrine and metabolic disorders based on their clinical features. Knowing the main laboratory and imaging tests required for the diagnostic procedure. These objectives will be achieved through lectures, seminars and interactive teaching activities with presentation and discussion of clinical cases, designed to facilitate the learning process and to improve the student ability to identify and deal with the main endocrine and metabolic disorders.

• Principles of endocrinology: definition of endocrinology, hormone synthesis and secretion, feedback mechanisms regulating hormone synthesis and secretion, hormone transport, hormone receptors, hormone actions. • Pathophysiology of the hypothalamus-pituitary axis, disorders of the hypothalamus and pituitary gland: pituitary tumors, prolactinomas, acromegaly and gigantism, diabetes insipidus, hypopituitarism. • Pathophysiology of the hypothalamic-pituitary-thyroid axis and thyroid disorders: thyroiditis, hypothyroidism, hyperthyroidism, thyroid cancer. • Pathophysiology of calcium-phosphorus metabolism and disorders of the parathyroid glands: hypocalcemia, hypercalcemia, hypoparathyroidism, hyperparathyroidism. Osteoporosis and metabolic bone diseases. • Pathophysiology of the endocrine pancreas, type 1 diabetes mellitus, type 2 diabetes mellitus, gestational diabetes mellitus, acute and chronic complications of diabetes mellitus, hypoglycemia. • Pathophysiology of the hypothalamic-pituitary-adrenal axis: adrenocortical hypofunction and adrenocortical insufficiency (Addison’s disease), adrenocortical hyperfunction, Cushing syndrome, pheochromocytoma (chromaffin cell tumor of the adrenal medulla). • Pathophysiology of the hypothalamic-pituitary-ovarian axis: hypogonadism, menstrual disorders and amenorrhea, hyperandrogenism and polycystic ovary syndrome, female infertility. • Pathophysiology of the hypothalamic-pituitary-testicular axis: hypogonadism, cryptorchidism, male infertility. • Obesity, metabolic syndrome and dyslipidemias.
